Request Non-Identifying Information
If you are seeking background and medical information about your birth relative and do not wish to identify or contact them, the following options are available. Please be aware that the amount of information in The Cradle's records varies according to the length of time we worked with the birth parent(s) and the landscape in the era in which the adoption was planned. Illinois adoption law does not allow The Cradle to release names and other identifying information without written consent
Background History Report
Adopted adults and/or adoptive parents may request that The Cradle provide all of the available non-identifying information of birth relatives as recorded at the time of adoption planning. Information may include: age, physical description, religion, national descent, educational, occupation, marital status, birth parents' interests, birth parents' relationship,and the circumstances surrounding the decision to make a plan of adoption, as well as communication with The Cradle after placement. We are also able to provide any medical information we have in our records. This information may include diseases within the birth family, a description of the pregnancy, facts about the labor and delivery, records of time spent in the Cradle nursery and any subsequent medical developments of which we have been notified. Brief Update Report
If you have received non-identifying information in the past you can request this report to check for any updated information or contact from the birth family since the original request. We will also provide an update on any contact with birth family members following the adoption. Get Started
Birth Parent Report
For birth parents, The Cradle can provide basic non-identifying information about your child's adoptive family at the time of the adoption. This information most often includes: age, physical description, religion, national descent, educational level, occupation, marital status at the time of the adoption, information on your child's early development and contact with The Cradle after placement.
