  (The medical questionnaire is valuable to fill out so that even if you do not wish contact with your biological relative/birthparent, you can provide updated health information. Often, files from decades past do not reflect current medical information.)

Search Fees

Regardless of the outcome, the search fee is non-refundable. Counseling/Consultation are included in the search fee as needed to help with all of the thoughts, feelings, and emotions that can result from this process.
