Adoptive Parent Reference Forms

We are considering your employee/friend/family member for the adoption of a child. To assist us, please offer your impressions of them as prospective parents. Your comments will be confidential, and we will not use your personal information for any purpose beyond contacting you about the reference you completed. Your reference will be emailed to a Cradle counselor, and a copy will be emailed to your email address.
